Clinicoradiologic Fearures Of The Zigomaticoorbital Complex Fractures In Polytraumas
Abstract
Research objective: to study the clinicoradiologic features of the zygomaticoorbital
complex fractures associated with polytraumas.
Material and methods. In this research, we studied the clinicoradiologic features of the
zygomaticoorbital complex fractures in 117 patients with polytraumas treated in the
Departments of Neurosurgery, Plastic Surgery of the Multidisciplinary Clinic of the
Tashkent Medical Academy. All patients with suspected traumatic injuries of the midface
zone and orbit underwent an ophthalmological examination and MSCT.
Results and discussion. The research revealed that in cases of ZOC fractures the most
injured areas of the orbit were orbital floor (82.1%) and lateral wall (55.6%). Orbital floor
and lateral wall fractures often resulted in the development of complications such as diplopia
(42.7%), hypophthalmos and enophthalmos (12.8%). The incidence of isolated injuries of
the orbital floor associated with ZOC fractures was 29.3%. Besides, we developed patient
management tactics consisting of two main directions: surgical treatment with the choice of
an implant in accordance with the calculated volume and defect area, and prosthetics of the
orbital floor with implants in accordance with the defect size with inclusion of metal osteosynthesis elements in the midface skeleton zone.
Conclusion. The data obtained proved that the informative value of MSCT, taking into
consideration the modeling and adaptability of the implant, was highly informative in
diagnostics and accurate for choice of surgical treatment tactics
